Model of care
The Nursing Model of Care at the Lehigh Valley Hospital–Hazleton is a team-oriented approach. The "Know Your Patient Model of Care" is a representation of nursing practice in which a qualified professional nurse is responsible and accountable for the nursing care of an assigned group of patients.
The RN charge nurse is responsible for making the assignments for the oncoming shift, and the assignments are comprised of RN/RN or RN/LPN teams who deliver care to an assigned group of patients in order to allow the nurses the opportunity to work closely with a specific group of patients so that they can get to "know the patients" who are assigned to their care.
RNs receive reports on their assigned patients whenever they come on shift and do regular rounds on their patients in order to assure the proper level of care and tend to any patient needs. Bed assignments are made by the nursing supervisor on duty at the time of admission.
Nursing vision statement
Our nursing process emphasizes quality and individualized patient care. Nursing involves patients, families and/or significant others in all aspects of care — we put patients' needs first. We recognize the right of all individuals to be a full partner in their care. Because of this collaborative nurse/patient relationship, we have made an ongoing commitment to providing service, quality, integrity, respect and caring to all individuals regardless of age, race, sex, sexual preference, creed or national origin and to assuming a leadership role in promoting excellence in health care within the community.
