LVHN ACO Preferred Partners
LVHN ACO has provided a list of preferred providers for beneficiaries.
This list does not limit the provider that beneficiaries are permitted to see. Beneficiaries always have a choice about what doctors they see or hospitals they visit.

The SNFs designated as Preferred or Aligned have signed a cooperative agreement with LVHN.
SNFs were evaluated based on:
Quality
- Performance in key quality metrics and patient outcomes
- CMS quality ratings and
- Readmission rates
Care coordination
- Patient volume
- location of service and
- On-site LVHN/LVPG provider presence
Cost factors
- Length of stay
- Resource Utilization Group (RUG) and
- Cost per episode
Patients will receive information about these partnerships from hospital Case Managers. Patients may choose any SNF, even if not Preferred or Aligned with LVHN. LVPG Geriatrics, through SNF Learning Collaboratives, will continue to work with all facilities.
However, pilots for any new pathways and protocols will be piloted with Preferred facilities and then implemented with all tiered facilities. Evaluations of facilities will occur annually and their relationships with LVHN may change as a result.
