Focus on Independence

All our patients receive at least three hours of therapy a day, for a minimum of five days a week. Your rehabilitation program will be customized to meet your specific needs and abilities, with a goal of increasing your independence as much as possible. You’ll be encouraged to:

  • Actively participate in your individualized therapy program
  • Join in group activities that help support your therapy
  • Participate in daily care activities such as dressing and grooming
  • Eat your meals with our other patients in the dining room

Rehabilitation is a team effort. So while you’re working on restoring function and building independence, your family will learn how to support you and take an active role in your ongoing recovery.

Home-like Features

Our 16-bed state-of-the-art center is located on the sixth floor of Lehigh Valley Hospital–Hazleton. Because our goal is increasing your independence, we want you to feel at home. The comfort starts with your room and extends to the bright and spacious therapy gym. Fully-functional kitchen, laundry and living areas allow your therapists to work with you in real-life settings so you can get back to doing things for yourself. Take a look inside:

Interdisciplinary Team

Your rehabilitation program will focus on both your physical needs and emotional well-being. It takes a team of specialists with many different skills and backgrounds to provide that kind of well-rounded care. Our care team includes:

  • Physiatrists (rehabilitation physicians)
  • Rehabilitation nurses
  • Physical, occupational and speech therapists
  • Psychiatrist
  • Internists (internal medicine physicians)
  • Dietitians
  • Social workers/case managers

Patient Referrals

Our center accepts referrals from physicians, social workers, nurses, case managers, discharge planners and other care providers. You or your family member can ask your physician or hospital social worker to initiate a referral.
