How to Add Care Planning Documents to your MyLVHN Account

MyLVHN allows you to upload and store care planning documents so that your health care team can easily access them when needed. For the best viewing experience, use the desktop version of MyLVHN. Follow the instructions below and you can safely store your advance directive, living will, power of attorney and other important documents.

  1. Once logged into your MyLVHN account, go to the menu at the top left of the main page.
  2. Scroll to “My Record,” and then click “Advance Care Planning.”

  3. On the “Advance Care Planning” page, find the “Planning Documents” section and click “Add a Document.”

  4. When you click on “Add a Document,” you will be able to browse your computer files and upload the document you would like to add.
  5. Simply click on the document and it will upload.
  6. Please note that you cannot remove documents yourself. You will need to message your care team to delete a document for you.

When using the Pennsylvania Advance Health Care Directive from LVHN, download the form and with Adobe Acrobat Reader, select “fill and sign” on the right side of the screen. You can then complete the form electronically.  You can also download the form, print it out, complete it by hand and photograph the pages. These can then be uploaded using the instructions linked above.
