How to Apply

Please submit the following materials through the Pharmacy Online Residency Centralized Application Service (PhORCAS) by Jan. 3:

  1. Letter of intent
  2. Curriculum vitae
  3. Official pharmacy school transcripts
  4. Three references
  • Two references must be clinical preceptors.
  • One additional reference must be the PGY1 Pharmacy Residency Program director.
  • References must use the Standardized Reference form available in PhORCAS.
  • References must provide and upload a traditional letter of recommendation in PhORCAS.

Please complete the following application requirements by Jan.3:

  1. Lehigh Valley Health Network online employee application
  2. Register with PhORCAS and the National Matching Service. 
     

Individual on-site interviews are required and will be scheduled by invitation.
