The Connections Program provides a compassionate approach to prenatal through postnatal care with comprehensive case management for pregnant women who have substance use disorder.
Our goal is to support women with substance use disorder and connect them to services to ensure an optimal pregnancy outcome, resulting in a healthy infant living in a positive family environment.
This program occurs in the supportive setting of your OB-GYN office where you will receive:
- Prenatal care for you and your baby
- Education about your needs and those of your baby
- Care coordination and linkage to treatment beyond your pregnancy needs, including counseling and medication to aid in your recovery if appropriate
- Referrals to supportive community resources, treatment and counseling services
- Peer support with a certified recovery specialist
- Plan of safe care education and development if appropriate
- Continued medical care and outreach after your delivery
- Advocacy for you and your baby
