At Lehigh Valley Health Network, we are dedicated to helping you live your happiest and healthiest life while managing chronic conditions. As part of that mission, we have developed the Diabetes and Metabolism Center (DMC), which provides comprehensive care and education services to adults with diabetes and prediabetes, as well as nutritional support for those with obesity, chronic kidney disease and other metabolic conditions.
What to expect
The DMC provides you the opportunity to work closely with certified diabetes educators and health coaches. If you have an uncontrolled A1C, you also can work with a clinician to try different medical therapies. With a variety of classes and individualized counseling offered, you can find a service at the DMC that fits your needs and your schedule.
Education
You will have the choice of participating in group or individual diabetes education classes available both in person and virtually. Topics covered include healthy eating, exercise, medication management, risk reduction and stress management.
Care Companion
You will have access to additional diabetes educational resources through your MyLVHN account.
Medical nutrition therapy
We offer individual nutrition counseling to help you with any of these conditions or concerns:
- Diabetes
- Prediabetes
- Chronic kidney disease
- Obesity
- GI disorders
- Tube feeding
- Cardiac disease
- Eating disorders
Diabetes educator visits
Get individualized education on daily lifestyle management strategies for living with diabetes. This includes:
- Problem-solving and counseling
- Preventing and managing complications
- Intensive insulin therapy management
- Insulin pump and continuous glucose monitoring training
Lifestyle management
You will have the opportunity to meet with a lifestyle medicine clinician as well as work with our health coaches. This focuses on:
- Nutrition
- Sleep
- Exercise
- Stress
- Avoidance of risky substances
- Relationships
Medication optimization
If you have uncontrolled diabetes and enroll in this program, you can work with one of our clinicians to optimize your diabetes medications. You will meet with the clinician monthly for a short period of time (typically three to six months) and are supported in this process with a dedicated nurse navigator who will help you set up appointments, check in with you on your progress and answer any questions you have throughout every stage of your care journey. Our clinicians work closely with your primary care doctor during this time to help you reach your A1C goals.
Remote patient monitoring
We use technology such as continuous glucose monitors when appropriate to help you achieve your goals.
Prediabetes management
LVHN can help people with prediabetes learn how to delay or prevent the onset of type 2 diabetes. You’ll learn about healthy eating and exercising, in addition to receiving information about diabetes and why prevention is so important.
Goals and timeline
The DMC care team’s main goal is to help educate and empower you to take control of your diabetes and reach your A1C goals.
Most people reach this stage within three to six months; however, if additional support is needed your clinician will discuss that with you at your visits.
Throughout your time in the DMC, your care team will stay in close contact with your primary care clinician, updating them on your progress and any changes to your medication.
