Sometimes you or your loved one may need additional care after the hospital. That’s where LVHN’s PACC can help. The PACC is a coordinated effort among various health care providers and facilities to manage and improve the transition and care of patients after they have been discharged from an acute care hospital. This collaboration typically includes a variety of post-acute care providers such as skilled nursing facilities, inpatient rehabilitation facilities and home health agencies. The members of a PACC aim to provide high-quality, patient-centered care that is both effective and efficient, ultimately leading to better health outcomes and enhanced patient satisfaction.
Continuing your recovery
Our goal is to give you the information you need to make the best choice for you and your family, caregiver or support person. When you or your loved one’s physician advises further care after an illness, injury or surgery, you may choose any provider that can best meet your care needs. The PACC makes it easy for you to find the continued care you trust during your recovery at home or at a location near you.
Two ways to continue care
Home care
If you or a loved one require specialized care as a result of illness, injury or a chronic condition after a hospital stay, you may qualify to receive skilled nursing and rehabilitative services from the comfort of your own home rather than in a post-acute care facility.
Skilled nursing facility
A skilled nursing facility provides 24-hour skilled nursing care and rehabilitation services, in addition to support from social workers, dietitians and certified nursing assistants under the supervision of a physician.
