What is home care?

If you or a loved one require specialized care as a result of illness, injury or a chronic condition after a hospital stay, you may qualify to receive skilled nursing and rehabilitative services from the comfort of your own home rather than in a post-acute care facility. Skilled home care requires a doctor’s order and refers to medical and rehabilitation services provided by licensed health care professionals in a patient’s home.

Lehigh Valley Health Network partners with home health agencies that have been chosen for their focus on quality care, working together to continue care and improve your recovery. These partnerships form the PACC. Having options allows you or your loved one to choose a trusted partner to care for you during the rest of your recovery.

Home health agencies that are part of the PACC have high quality-star ratings from the Centers for Medicare & Medicaid Services. This means you’ll receive the excellent care you deserve and get back to feeling better faster. Quality ratings for these agencies are online at medicare.gov.

Helpful tips for choosing a PACC home care provider

Please call your insurance provider to ensure the facility you choose is covered. Some insurance companies have contracts with specific providers, suppliers and facilities, which may affect the post-acute provider or services you can use.

If you choose a post-acute provider or service that is not covered under your insurance plan or not participating in Medicare, you could be liable and might have to pay for the costs involved.

If you are enrolled in a managed care organization, you should check whether your chosen provider is covered by your insurance plan before you pick that provider.
