How to Schedule Your Child's COVID-19 Vaccine Appointment

LVHN is now offering COVID-19 vaccination appointments for children age 5 and older. To schedule an appointment through MyLVHN, you will need proxy access to your child's MyLVHN account. If you don't already have proxy access, you will need to request it by calling your child's provider's office. 

You can also schedule an appointment by calling our COVID-19 Hotline at 833-584-6283 (833-LVHN-CVD) Monday-Friday, 8:30 a.m.-5:30 p.m., or if your child is age 12 and older, by going to LVHN.org/vaccines.
