LVHN Cares for People With COVID-19 Through Remote Patient Monitoring

At Lehigh Valley Health Network (LVHN), more than 1,000 COVID-19 patients have been discharged from our hospitals. Most patients feel an overwhelming sense of relief when they leave the hospital because it’s a significant step toward recovery. However, it’s common to have concerns: What if my temperature spikes? What if my shortness of breath persists? What’s normal during recovery and what needs special attention?

Whether you are headed home from the hospital or you just tested positive for COVID-19, Continuous Ambulatory Remote Engagement Services (CARES) now offers remote patient monitoring for COVID-19 patients. If your condition changes, your health care team will know and will be able to provide guidance quickly. That means, you can be confident that LVHN is with you every step of the way.

How does it work?

If you are enrolled in the COVID-19 CARES program, you’ll receive a kit with tools to record your blood oxygen levels and temperature. Then you will log into MyLVHN to record those data points and answer questions about your symptoms twice a day. You can even request a call from a CARES nurse.

Your information is reviewed by a nurse on the LVHN CARES team. If needed, the nurse will reach out to discuss your condition or provide the information to a physician for further review.

LVHN CARES

There are 124 people currently enrolled in COVID-19 CARES and more than 1,000 people enrolled in all CARES programs. Simply put, CARES prevents readmissions to the hospital. It allows the LVHN team to react quickly to people’s symptoms before their condition requires hospitalization. It’s just one more way LVHN provides the best care.
