Welcome to Lehigh Valley Health Network! We are excited to host you for your clinical or field experience. To be approved to begin your experience, please review the following onboarding requirements and complete the required new student documentation.
InPlace:
Lehigh Valley Health Network (LVHN) has partnered with InPlace to standardize the placement process. All placement requests and student requirements must be uploaded through this software system.
For schools utilizing InPlace: All placement requests must be submitted through InPlace at least four weeks prior to the start date. Once approved by LVHN, all student documentation must be uploaded to InPlace at least two weeks prior to the start date.

Please review all LVHN requirements and get access to the orientation modules.
To be approved to begin, all requirements must be completed and submitted to LVHN at least two weeks prior to the beginning of the student experience. If required documentation is not uploaded into InPlace scheduling software system at least two weeks prior, the student start date will be delayed until all required documentation is complete.

Criminal Clearances
All students completing a clinical rotation or field experience must complete criminal background checks and submit the information two weeks prior to beginning their educational experience at LVHN, pursuant to Background Screening–Human Resources Policy.
The process for completing these clearances is as follows:
- Student completes online request for Pennsylvania State Police Criminal History Report (PATCH form). The purpose of this check is: Employment/Screening.
- Student completes Child Abuse History Clearance (CAHC) application. Students can view/print report directly from the Pennsylvania child welfare site. (Please select application purpose: “14 years of age or older …”)
- Student requests Department of Human Services FBI Fingerprint Clearance, consistent with the Child Protective Services Law. Please click here for instructions.*
- GRADUATE NURSING STUDENTS ONLY must also submit a copy of RN license and a copy of professional liability insurance.
*Instructions for Pennsylvania Fingerprinting Clearances
The approved vendor for processing for the Pennsylvania Department of Human Services FBI Fingerprint Clearance, consistent with the Child Protective Services Law, is IdentoGO.
We suggest that all students begin registering for fingerprinting at least 6-8 weeks prior to rotation start date.
A few points to remember:
- Students are responsible for paying for their own clearances.
- Clearances take 7-14 days to process once fingerprints are submitted.
- Department of Human Services help desk is: 800-692-7462.
- IdentoGO help desk is: 855-845-7434.
To request either or both clearances, follow the step-by-step instructions below:

- Step 2: Register for the Department of Human Services (DHS). Begin by entering service code: 1KG756. At the top of the page, you should see “Employee >= 14 Years Contact with Children.”
- Step 3: Follow the directions on the website: https://uenroll.identogo.com/
- Step 4: Begin registration by entering the service code: 1KG8RJ
- Step 5: Follow the directions on the website. The LVHN Facility Code is “1015.”
The agency/agencies will mail you a copy of your clearance report. Medical students can upload a copy of the report(s) to VSLO. All other students can upload them to their InPlace account.
