Lehigh Valley Orthopedic Institute is committed to partnering with you for a safe return to athletic participation. Please take advantage of our PIAA sports physical self-scheduling options through our secure, electronic portal: MyLVHN.
We are scheduling PIAA sports physicals at our Lehigh Valley Health Network (LVHN) ExpressCARE locations throughout the region. Learn about our strict cleaning protocols how LVHN is keeping patients safe in our facilities.
Before your PIAA sports physical
Before your appointment, please print all 10 pages of the PIAA sports physical and complete sections one through five in advance. The form must be completed not earlier than six weeks prior to the first practice day of the sport(s) in the sports season(s) identified. You will need to bring your completed PIAA sports physical form with you to your appointment.
Schedule your PIAA sports physical through MyLVHN
If you are a current patient of LVHN, please schedule your PIAA physical through the MyLVHN portal.
- Visit MyLVHN.org and sign in. If you are a parent or guardian to an athlete, make sure you have proxy access.
- Click “Visits” located in the top toolbar.
- Click “Schedule an appointment at ExpressCARE” and select PIAA Sports Physical as the reason for appointment.
- Select a location and time.
- Confirm appointment.
If you do not have an account, you may enroll in MyLVHN and follow the steps above.
If you would like to schedule a PIAA sports physical without a MyLVHN account, please complete this secure MyLVHN form. You will be contacted by a member of our sports medicine team by phone to confirm an appointment time and location.
Proxy access information
If you are a parent or guardian to a child or another person, request proxy access to their health information. Proxy access lets you schedule appointments and set up a PIAA sports physical on their behalf. In order to set up proxy access, you will also need your own MyLVHN account. All health information will be recorded in your child’s LVHN medical record.
If you currently do not have access to your child’s MyLVHN account, you will need to contact your child’s provider to request proxy. Establishing proxy access to your child’s MyLVHN account usually happens at your child’s appointment.
If your child does not have an LVHN primary care provider, please call 888-402-LVHN (5846) for assistance.
Unfortunately, if the proxy access is not already established, you cannot schedule a PIAA sports physical for your child. Please contact your child’s health care provider to request proxy access so it is available when you need it.
Tip: If you see your child’s or loved one’s photo on your MyLVHN account, you have proxy access.
