Emergency Medical Services Application

Fellows must be board-certified or board eligible and have successfully completed an ACGME-accredited residency program.

Requirements include:

1. Personal statement
2. Three letters of recommendation (one must be from your program director)
3. USMLE and/or COMLEX board scores
