Summary

Facilitates and advises internal quality improvement and quality assurance teams. Identifies opportunities for clinical and/or process improvement and facilitates needed changes to improve care. Acquires, validates, interprets, and transforms data into actionable information.

Job Duties

  • Facilitates and advises internal quality improvement teams. Identifies opportunities for clinical and/or process improvement and facilitates needed changes to improve care.
  • Facilitates and advises Quality Assurance Committees. Analyzes peer review case findings to identify opportunities for improvement.
  • Utilizes project management skills such as Gantt charts, communication plans, process flow diagrams, team building, conflict resolution, problem solving, and facilitation intervention to foster team development and progress toward goals.
  • Incorporates evidence-based best practices or established organization guidelines around proposed interventions.
  • Utilizes appropriate and acceptable intervention behaviors and communication styles in order to effectively facilitate change and process/project improvement.
  • Serves as a resource to network teams to determine data/information needed to measure processes and outcomes. Facilitates the creation of dashboards or appropriate reports.
  • Summarizes data in a clear, concise format for presentation and utilizes appropriate data visualization methods.
  • Ensures alignment and integration of the improvement initiative with applicable regulations, standards, and guidelines from standards setting and accrediting bodies.
  • Demonstrates knowledge of public reporting requirements and assists with processes and improvements needed to report and improve publicly reported clinical outcomes.

Minimum Qualifications
  • Bachelor’s Degree
  • 3 years performance improvement experience including group dynamics, change management, data retrieval, and analysis. and
  • 3 years clinical patient care experience.
  • Ability to provide leadership and coordinate team activities to meet quality goals.
  • Ability to manage multiple projects simultaneously using traditional project management systems and processes.
  • Demonstrates advanced knowledge of clinical information available in hospital database applications.
  • Computer literacy and database/data mining skills (statistical concepts, data management, variance tracking, and analysis).
  • Expertise in accreditation standards and patient safety improvement concepts.
  • High degree of self-direction and knowledge of quality and performance management concepts.
  • Knowledge of risk adjustment models and ability to utilize risk-adjusted data to support improvement initiatives.
  • Demonstrates motivation to learn through independent reading, professional networking, and attending relevant conferences and webinars.
  • CPHQ-Cert Professional Healthcare Quality HQCB - State of Pennsylvania within 2 Years

Preferred Qualifications
  • Master’s Degree

Physical Demands

Lift and carry 25 lbs. frequent sitting/standing, frequent keyboard use, *patient care providers may be required to perform activities specific to their role including kneeling, bending, squatting and performing CPR.
