Summary

Assists in supervision, orientation, and coordination of the LVHN Inpatient Care Management staff to ensure adequate coverage of the department and competency of staff.

Job Duties

  • Assists in the development and implementation of policies/procedures/standard work designed to facilitate positive patient outcomes.
  • Coordinates schedules and determines daily assignments.
  • Creates and supports a work environment that encourages and empowers others to contribute according to their strengths and abilities.
  • Ensures that new employees are meeting goals and establishing competencies through chart audits.
  • Analyzes and interprets reports related to productivity and staff compliance.
  • Oversees orientation of new employees by developing a schedule for training and establishes orientation procedures and reviews annually
  • Prepares required written reports with correct grammar and format.
  • Provides input to Director budgetary needs.
  • Provides ongoing communication with appropriate leaders, staff, and agencies to facilitate collaboration.
  • Implements service excellence strategies to improve and/or maintain patient satisfaction at an acceptable level.

Minimum Qualifications
  • Bachelor’s Degree in nursing or
  • Master’s Degree in social work, nursing, healthcare administration, or related degree. or
  • Bachelor’s Degree with 5 years of acute care management experience and one year as a lead CM at LVHN
  • 2 years experience in acute care management.
  • RN - Licensed Registered Nurse_PA - State of Pennsylvania Upon Hire

Physical Demands

Lift and carry 40-50 lbs. Examples: Push/pull patients on bed, stretcher (requires 29 lbs. push force), lateral transfers up to 50 lbs. of the patient's weight. Frequent to continuous standing/walking. Patient transporters can walk 8-10 miles per shift. *Patient care providers may be required to perform activities specific to their role including kneeling, bending, squatting and performing CPR.
