Summary
Coordinates the services for solid organ transplant patients and communicates with the multidisciplinary patient care team, nephrologists, and dialysis centers, staff, and other healthcare providers.
Job Duties
- Daily care of transplant patients in the outpatient clinic.
- Coordinates care and discharge of patients from the inpatient setting.
- Reviews medical history, coordinates schedules for medical testing, and reviews results to ensure optimal outcomes.
- Obtains and reviews referral information from referring and consulting providers and other referral sources.
- Ensures completion of appropriate testing and provides results to physicians and advanced providers
- Participate in patient education and discharge planning in the pre and post operative setting
- Facilitates development and management of clinical protocols
- Participate in on call duties for the transplant program on a rotating basis
- Collaborates with the Organ Procurement Organization and on call surgeons to determine suitability of donor organs.
- Manages admission of potential donor and recipient to hospital for surgery.
Minimum Qualifications
- Bachelor’s Degree in Nursing
- 4 years Experience as a nurse in the medical, surgical, and or dialysis field.
- Demonstrated knowledge of current transplant-related medical information.
- American Heart Association Basic Life Support - State of Pennsylvania Upon Hire
- RN - Licensed Registered Nurse_PA - State of Pennsylvania Upon Hire
Preferred Qualifications
- Master’s Degree in Nursing
Physical Demands
Lift and carry 40-50 lbs. Examples: Push/pull patients on bed, stretcher (requires 29 lbs. push force), lateral transfers up to 50 lbs. of the patient's weight. Frequent to continuous standing/walking. Patient transporters can walk 8-10 miles per shift. *Patient care providers may be required to perform activities specific to their role including kneeling, bending, squatting and performing CPR.
