Today, having rapid access to information is a given. With MyLVHN, you can easily stay connected to your health information and in contact with your care team.
MyLVHN is the patient portal for all Lehigh Valley Physician Group (LVPG) practices, Lehigh Valley Hospitals, and LVHN outpatient services. (See information below concerning former patient portals.)
With MyLVHN you can:
- Review medical information
- See test results quickly
- Schedule an appointment
- Have a video visit or an E-Visit
- Request prescription refills
- Pay or review bills for medical services (you can pay as a guest too)
- Send and receive secure, non-urgent messages from your LVHN health care teams
Get access to MyLVHN
MyLVHN is available as a website as well as a smart phone app.
- Website: From a computer connected to the Internet, visit MyLVHN.org.
- App: From your mobile device, download the free MyLVHN app (available for Apple or Android devices).

Having the MyLVHN app is important for accessing features like video visits and other video screenings.
How to enroll in MyLVHN
There are a few ways you can enroll in MyLVHN, with or without an activation code.
Proxy access information
If you are a parent or guardian to a child or another person, request proxy access at their next LVPG appointment. Proxy access lets you schedule appointments, check immunization records and set up LVHN Video Visits on their behalf. In order to set up proxy access, you will also need your own MyLVHN account.
Tip: If you see your child’s or loved one’s photo on your MyLVHN account, you have proxy access.
Out of town care
With MyLVHN, you have access to your medical history and almost all of the portal’s convenient features wherever you are – whether at home or on the road. (Note: To have a Video Visit, you must be in the state of Pennsylvania to receive care due to medical licensing requirements.)
If you are out of town and need to access medical care, your electronic health record can be made available to an authorized health care organization, helping to ensure you receive safe and comprehensive care. And should you receive care at another health care organization, you can link your MyLVHN account to them and then have the ability to see all of your health information in one place. Go to “Account Settings” in MyLVHN and select, “Link My Accounts.”
Former patient portals
Coordinated Health’s Follow My Health Patient Portal
As of Sept. 1, 2020, Coordinated Health’s Follow My Health patient portal is closed. You will now access patient information on MyLVHN. At your first visit beginning Sept. 1, 2020 and after, your health information will be confirmed and added to MyLVHN. You will need to create a new account at that visit or after if you do not have a MyLVHN account today.
LVH–Hazleton, LVH–Pocono, LVH–Schuylkill patient portals
As of Oct. 1, 2019, all former patient portals used by Lehigh Valley Hospital–Hazleton (MyLVHN Hazleton), Lehigh Valley Hospital–Pocono (Change Healthcare portal) and Lehigh Valley Hospital–Schuylkill (CPSI portals) have closed. You will now access patient information through MyLVHN. At your first LVPG visit beginning Oct. 1, 2019 and after, your health information will be confirmed and added to MyLVHN. You will need to create a new account at that visit or after it following the instructions listed above.
