The LVHN Employee Health Plan’s machine readable files (MRF's), required under the Transparency in Coverage Rule established by the U.S. Departments of Health and Human Services, Labor and the Treasury, may be accessed here. The files include negotiated rates that exist between the health plan and in-network providers (“In-Network Rates” files) for items and services covered by the health plan as well as historical data showing payment amounts for covered items and services furnished by out-of-network providers and paid by the health plan (called the “Allowed Amounts” files). The MRFs also contain a link to the negotiated rates in place with a leased network for claims that process for that leased network’s participating providers (available from this download for the “Table of Contents Files” section).
