Summary
Progresses patients care based upon medical criteria for in-patient status. Manages care and resources to reduce unnecessary utilization, facilitates outpatient testing and follow-up when appropriate and collaborates in the development and implementation of performance improvement activities in collaboration with care management, physicians, and utilization management.

Job Duties

  • Oversee a population of in-patients and progress care through discharge using evidenced based criteria.
  • Identifies patient clinical conditions, testing and value based clinical interventions (CARES/Hospital at Home/Complex Care Management) that can be completed in the ambulatory space.


Minimum Qualifications

  • Bachelor’s Degree in nursing.
  • Must have 5 years of ED experience; adult and pediatric emergency room experience
  • 5 years in-patient acute care hospital.
  • Excellent communication (oral and written) required to facilitate reviews and discussions with providers.
  • RN - Licensed Registered Nurse_PA - State of Pennsylvania Upon Hire


Preferred Qualifications

  • Knowledge of care and utilization management.
  • Strong interpersonal and organizational skills to manage case load and communicate with multi-disciplinary team members.
  • Ability to work independently and set priorities to ensure appropriate use of hospital resources.
  • Ability to lead individuals and groups.


Physical Demands
Lift and carry 40-50 lbs. Examples: Push/pull patients on bed, stretcher (requires 29 lbs. push force), lateral transfers up to 50 lbs. of the patient's weight. Frequent to continuous standing/walking. Patient transporters can walk 8-10 miles per shift. *Patient care providers may be required to perform activities specific to their role including kneeling, bending, squatting and performing CPR.
